- Origin : anterior tubercle of transvers process of vertebrae C3-C6
 - Insertion : First rib
 
 - Middle Scalene 
- Origin : posterior tubercles of transverse processes of vertebrae C2-C7
 - Insertion : First rib
 
 - Posterior Scalene
- Origin : posterior tubercles of transverse processes of vertebrae C6-C7
 - Insertion : Second rib
 
 - Action of All scalenes
- Bilateral contraction 
- Neck flexion
 - Elevate the ribs during inhalation
 
 - Unilateral contraction
- neck lateral flexion (ipsilateral)
 - neck rotation (contralateral)

Muscle testing

- Anterior Scalenes : slightly extend head –> contralateral flexion and ipsilateral rotation
 - Middle Scalenes : slightly extend head –> contralateral flexion
 - Posterior Scalenes : slightly extend head –> contralateral flexion and contralateral rotation
 
 - Stength test
- Anterior Scalenes : slightly flex head –>ipsilateral flexion and contralateral rotation –> resist each motion
 - Middle Scalenes : slightly flex head –> ipsilateral flexion –>resist that motion
 - Posterior Scalenes : slightly flex head –> ipsilateral flexion and ipsilateral rotation –>resist each motion

- Procedure 
- Fully rotate the head to the affected side
 - Neck flecion to the same side
 - pull the chin inferioly into the hollow hust posterior to the clavicle
 
 - Positive : Pain in the referred pattern for the scalane muscles
